The real problem turned out to be missing pan flashing — the chimney had to come down to the roofline and get rebuilt with a proper soldered copper pan turned in at the flues. No more leaks since. It's a good example of why a surface patch or a sealer coat rarely solves a chimney leak that's actually coming from inside the brickwork.
